Complete Our New Patient Intake Process
Follow the step-by-step process below to obtain a new patient appointment! All initial evaluations are conducted via telehealth and last 50 minutes. What's needed? A smartphone, desktop, laptop, or tablet, a cell phone number, an email address, and a good internet connection!

Step 1: Complete New Patient Request Form
To initiate your journey with us, the first step is to complete the new patient request form. This form will allow us to create your electronic health record and complete benefits verification.
Step 2: Activate Your Patient Portal
Once we receive the new patient request form and have validated your insurance, you will receive an email inviting you to activate your Patient Portal! Follow the step-by-step process to validate your identity and create your login credentials!
Step 3: Download The Patient Portal Mobile App
Download the MyRXNT Mobile App and use the username and password you created in Step 2 to log in. In the patient portal mobile app, you will be assigned a few consent forms for treatment and a health history form to complete electronically before your new patient appointment.
Step 4: Request Your New Patient Appointment
In the MyRXNT Mobile App, on the home page, select the bottom task bar, then select the calendar icon, and follow the steps to request a new patient appointment. You can request the soonest available time or choose up to 3 days and times that work best for you!
